Activity Description

Shared savings plans can mitigate health care costs, but how do we establish the relationships required for their success? Health plans can provide cost data demonstrating medication cost trends, but clinical data is required from the health system to offer a comprehensive perspective. Pharmacists have an integral role in bridging this gap to ensure health systems achieve clinical, quality and cost-of-care benchmarks. When a health plan pharmacist supports teams in established clinical practices, the likelihood of improving benchmark measures increases.

This session will demonstrate how interactions between health system and health plan pharmacists can improve clinical and quality outcomes in shared savings models. It will further describe why it is imperative to integrate community, hospital and PBM pharmacists into these interactions.

Learning Objectives:

At the completion of this activity, participants should be able to:

  1. Explain the differences between upside and downside risk in alternative savings models.
  2. Identify and implement strategies to manage risk in value-based payment models.
  3. Evaluate real world health plan and health system data to develop programs to help achieve desired benchmarks.
  4. Describe methods of pharmacist collaboration to improve outcomes in alternative payment models.
